At the Diabetes Learning Center, you’ll get one-on-one support tailored to your goals and your health. As the only American Diabetes Association (ADA)-recognized diabetes education program in South Georgia, we’re here to help you better understand diabetes and feel more confident managing it every day.

No group classes. No one-size-fits-all approach. Every session is individualized and focused on your specific needs, concerns and goals.

You’ll work with a nationally certified diabetes care and education specialist with advanced training in diabetes management and patient education.

Questions don’t stop after your first appointment, and neither do we. We’ll continue working with you until you feel comfortable and confident managing your diabetes.

Getting started
Start by getting a referral from your physician. From there, we’ll help you through the rest.

Your first visit
We’ll take time to learn about your health history, current treatment plan, concerns and goals.

Personalized education
Your one-on-one appointment typically lasts 90 minutes to two hours. Family members and caregivers are welcome to attend.

Follow-up support
Additional visits, including telehealth follow-up appointments, are available as needed.

Accessible Care for South Georgia
The Diabetes Learning Center serves patients throughout South Georgia, including Tifton, Cordele, Fitzgerald, Valdosta and surrounding communities. We accept Medicare and Medicaid, and financial assistance may be available for eligible patients. Spanish-speaking support and translation services are also available.

Do I need a referral?
Yes. A physician referral is required, but our team can help coordinate the process if needed.
Can a family member come with me?
Absolutely. Family members and caregivers are encouraged to participate.
Is diabetes education covered by insurance?
Many patients have coverage for diabetes education services. We accept Medicare and Medicaid, and can help answer questions about coverage.
How many appointments will I need?
Every patient is different. We’ll work with you for as long as you need support. Eligible patients may qualify for up to 10 hours of education during their first year.
Is telehealth available?
Yes. Follow-up visits may be available through telehealth when appropriate.
