Southwell Health and Rehabilitation is a hospital-based skilled-nursing facility that focuses on the physical and mental well-being of all residents, with an emphasis on warm comfort and quality care. Our goal is to enhance the quality of life for each patient and resident. We respect the individuality and personal story of all who are in our care.
Because we are a hospital-based facility, our residents have easy access to hospital services, such as radiology, laboratory, as well as our physicians. By being in close proximity to the hospital, our residents do not have to be transported to different facilities and can enjoy the benefits of staying in one place.
Our team of specialists, including licensed nurses, therapists, social workers and activities coordinators, establish a comprehensive and individualized treatment plan for each individual we serve.
