The Southwell Provider Portal is a patient co-management tool that gives you access to medical records for patients you have referred to Southwell. Get inpatient and outpatient visit data in real-time, including clinical notes, vital signs, lab and diagnostic test results, pathology reports and medications.
With Southwell Provider Portal, you can:
- Access clinical information about your patient
- Locate patients who have been referred to Southwell
- Review reported allergies, medications and problems
- Search for patients within the Southwell database

Provider Portal is a web-based tool that allows community providers to access medical records for patients who have been referred to Southwell. This tool displays information from inpatient and outpatient visits in real-time, including clinical notes, vital signs, lab and diagnostic test results, pathology reports, and medications. It is one way to improve communication between care providers and improve patient care.
Providers and their designated office staff can sign up for a Provider Portal account using an online form, Provider Portal Account Requests.
Our Legal department will reach out to you after receiving your request to provide more required documentation.
Contact our Health Information Management department at (229) 353-6610 for help with accessing patient information. HIM staff can associate providers with an encounter as needed or facilitate other requests for release of information. Please do not send patient information over unsecured email. Provider Portal also offers a search feature.
By law, some results are excluded from Provider Portal to protect patient privacy. Because the tool does not have an image viewer, scanned and other documents with images do not display correctly and are excluded from Provider Portal.
If you are searching for historical data, Provider Portal will display up to seven years of clinical data for a patient. Please contact the Health Information Management department at 229-353-6610 for data that is not available in Provider Portal.
