The Southwell Patient Portal is a convenient and secure health-management tool you can use anywhere you have access to the internet. Through the portal, you can:
- Correspond with your provider team
- Request an appointment with a primary care provider
- Request a prescription renewal from your provider
- Access your electronic health record
- Make an online payment
MySouthwell Patient Portal users can receive notifications via:
- SMS/text messages
- Mobile app push notifications
Notifications will be sent out when there are new:
- Secure messages
- Diagnostic results (lab, pathology, radiology)
- COVID-19 results
- Clinical notes
- Documents
Ready to Enroll?
There are two ways to enroll in the HealtheLife Patient Portal:
- Request an email invitation. Provide your email address to the Patient Access representative at your next visit with a Southwell/Tift Regional provider or facility. Or, you can call our Patient Access Department at 229-391-4174 to have an email invitation sent to you. The e-mail invite you will receive in your inbox will provide step-by-step instructions on how to register and set up your new HealtheLife patient portal account. Please note: protecting the privacy of health information is important to us, so we have made this process very secure. The email provided to you by Southwell/Tift Regional should be kept confidential.
- Self-enroll. You can also self-enroll on the HealtheLife Patient Portal by clicking on the link below. You will need your Medical Record Number (MRN). Please call your physician’s office to obtain your MRN in order to self-enroll in the Patient Portal. Or, you can request an email invitation be sent to you; see above.
We're an Apple Health Partner
Apple Health is an app with many health, wellness, and fitness features. It also allows you to consolidate and manage electronic health records on your iPhone or iPad from different participating healthcare organizations.
How to get started with Apple Health
Please note that adding electronic health records in the Apple Health app requires an iPhone with iOS 11.3 or later.
- Open the Apple Health app on your iPhone and tap the Summary tab
- Tap your profile picture in the upper-right corner
- Under Accounts, tap Health Records, then tap Get Started. To add another provider, scroll down to Accounts, then tap Add Account
- Search for Southwell, then tap it. Under the Available to Connect header, choose “Connect to Account"
- You will be redirected to your MySouthwell Patient Portal account. Sign in. You may be asked to save your password
- Wait for your records to update. It may take a minute for your information to appear
- Repeat these steps for any of your providers or hospitals outside of Southwell that the app supports
After adding an institution, the Apple Health app updates your health records periodically. If there’s an issue with your health records, please contact your provider.
View Your Records
- Open the Apple Health app, then tap the Browse tab
- Under Health Records, tap a category
- Tap an item to see more information about it
Remove a Health Record
- Open the Apple Health app, then tap the Summary tab
- Tap your profile picture in the upper-right corner
- Under Accounts, tap Health Records
- Tap the Provider that you want to remove
- Tap "Remove Account"
- To confirm, tap "Remove Account & Delete Data"
This also deletes the account and data from any other devices if you’re signed into iCloud and the same Apple ID.
