Cook Medical Plaza opened in 2019. The 120,000 square-foot campus hosts an array of medical services to meet the various needs of our community.  First is Southwell Medical, our state-of-the-art hospital, that includes acute care, laboratory services, sleep studies, medical imaging, endoscopy, an outpatient surgery center as well as physical, occupational and speech therapy services.

Southwell Medical also includes the Sylvia Barr Center, a 12-bed geriatric psychiatric facility that provides behavioral health services to adults who are aged 55 and older. 

Southwell Health and Rehabilitation is a 95-bed skilled nursing facility that is the newest and most modern facility in the region.  Because we are hospital-based, our residents have easy access to services, such as radiology, laboratory, therapy, and wound care, as well as our physicians. 

Please note that no emergency services are currently available on campus.  For emergencies, please dial 911.  When there’s urgency for treatment of a minor injury or illness, but not an emergency, please visit our convenient care clinic at Southwell Medical Adel Primary Care with extended weekday and weekend hours.
