Community Health Needs Assessment

Southwell is conducting a health needs assessment and is asking members of the community to participate in a web-based survey by November 23rd.

“The survey, which asks residents their thoughts on the health status of our community, only takes about 10 minutes to complete,” said Chris Efaw, Vice President, Outreach and Development with Southwell.  “The feedback we receive from area citizens will help shape the direction of medical care in our region.”

Efaw said that people can participate anonymously, but all participants who provide their contact information will be entered into a drawing for a $200 VISA gift card, a $100 Walmart gift card and a $50 Darden restaurant gift card (good at Longhorn, Olive Garden, and other locations). 

 “All responses remain confidential and are used for a blinded study only,” said Efaw.  “Southwell serves 14-counties in South Central Georgia, and we encourage residents throughout our entire region to participate.”

Efaw said the purpose of the assessment is to identify community health needs, which helps guide Southwell in strategic planning and the prioritization of resources.

Conducted by an independent third party, the health needs assessment includes the community survey, demographic research, hospital discharge data analysis, and interviews with physicians, clinicians, and local opinion leaders.  Southwell updates its health needs assessment every three years in accordance with federal requirements for all 501C3 nonprofit hospitals and healthcare systems.

Southwell includes Tift Regional Medical Center (TRMC) and the TRMC West Campus in Tifton, Southwell Medical and Southwell Health and Rehabilitation in Adel, and numerous physician practices located throughout South Central Georgia.

People without internet access can call 229-353-6318 or 800-648-1935, ext. 6318 to request that a survey form be mailed to them.   A self-addressed, self-stamped envelope will be included so the completed form can be easily returned.   

The survey closes-out on Nov. 23 and the prize drawing will be held on Nov. 24.  Winners will be notified.
